Q-ACCIDENT - Accident / Incident / Condition Reporting

Getting consistent reporting of accidents, incidents, near misses and other adverse safety conditions can take time and effort. So many questions …

  • What type of event was it?
  • Where did it happen - in what part of the site, or if it was off-site, where?
  • Who was involved?
  • Was that injury a cut, slash, gash, incision, slice,….?
  • Where on the body did it happen?
  • Who were the witnesses?
  • What information have they provided?
  • Has that person been involved in any previous incidents?
  • Was there any hazardous work taking place at the time?

And so many more…

Getting an IT system in for it can seem like a good answer – but so many of them make the process even harder and more time-consuming.

Q-ACCIDENT provides a simple user interface for the entire process.

Reporting injuries, for example, is a simple drag and drop – drag an injury to the location on the body where it happened – and drag it around to adjust it afterwards.

And as you let go of the drag/drop, you get an immediate menu indicating how serious it was.

Simple, quick, consistent.  And as you simply drag and drop, the incident is written up in English for you.

More complex cases with multiple injuries are just as simple. Someone was injured, with bruises to the shoulder and a dislocated wrist? The bruising was treated onsite, but the dislocation took them to hospital?  Simple – drag “bruise or localised impact”  onto the shoulder, and specify “onsite first aid”. Then drag “dislocated joint”   onto the wrist (it will only drag to joints) and specify “taken to hospital”.

Just a few seconds for a precise and consistent description of the injuries, their locations and their respective severity.

Q-ACCIDENT takes you right from the initial report through to an in-house report and RIDDOR report.

Reporting and analysis are just as easy - you define a list of reporting categories,  and the values that each can take.    When an incident occurs, you specify the values that apply.  And the system automatically produces analyses broken down by those categories over time,  either for a single site or over all your sites.
